Wisdom tooth removal at the Ghatkopar East clinic

Wisdom teeth are removed when they cause repeated infection, decay, damage to the tooth in front, or a cyst. Straight, cleanable wisdom teeth are left alone. Surgical and impacted removals are done under local anaesthetic at the Ghatkopar East clinic, with an OPG or CBCT first.

First question: does it actually need to come out?

Not every wisdom tooth does. If it has come through straight, bites properly against the tooth above it, and you can clean it, leave it alone. Removing a healthy, functional wisdom tooth is surgery you did not need.

When it does need to come out

  • Repeated infection round the gum flap. Pericoronitis. Swollen, painful, foul taste, hard to open your mouth. Once it has happened twice it will keep happening.
  • Decay you cannot reach. Either in the wisdom tooth or, worse, in the healthy second molar it is pressed against. That second tooth is the one worth saving.
  • It is pushing into the tooth in front and damaging its root.
  • A cyst forming around a buried tooth, seen on an X-ray.
  • It bites into your cheek or has no opposing tooth and has over-erupted.

Impacted, and what surgical removal means

An impacted wisdom tooth is stuck, either under the gum, under bone, or angled into the tooth in front. Horizontal impactions are the most common reason a removal becomes a surgical one.

Surgical removal means a small incision, sometimes a little bone removed around the tooth, and the tooth sectioned into pieces so it comes out through a smaller opening rather than being forced whole. Sectioning sounds worse and is actually gentler. Stitches usually follow, often dissolvable.

Why the X-ray is not optional

In the lower jaw the roots of a wisdom tooth can sit against the inferior alveolar nerve, which supplies feeling to your lip and chin. An OPG shows the relationship, and where it looks close, a CBCT scan shows it in three dimensions.

This is the single most important part of planning the surgery, and it is what separates a considered extraction from an optimistic one. Any clinic that offers to take out a lower impacted wisdom tooth without imaging it first is guessing with your lip.

The appointment itself

Local anaesthetic, and you are numb before anything starts. A straightforward removal takes minutes. A surgical one usually runs twenty to forty minutes. You will feel pressure, which is normal, but you should not feel sharpness. Say so if you do and more anaesthetic goes in.

Sedation is available if the idea of it is the problem rather than the procedure. Plenty of adults need that and there is nothing unusual about asking.

Recovery, honestly

  • First 24 hours. Bite on the gauze. Do not rinse, spit, use a straw or smoke, all of which dislodge the clot. Cold compress on the outside, twenty minutes on, twenty off. Eat soft and cool.
  • Day two and three. This is usually the peak of the swelling, not day one. Start warm salt water rinses. Keep taking the painkillers on schedule rather than waiting for pain to return.
  • Day four onward. Swelling settles, opening improves. Most people are back to normal food within a week.

Dry socket is the complication to know about. If the pain gets worse rather than better around day three and radiates to the ear, the clot has been lost. It is treatable in one short visit, so come in rather than pushing through.

Planning around work

The clinic runs until 9 pm, six days a week, so a straightforward removal can be done on a Friday evening or a Saturday and you lose no working days. For a difficult lower impaction, take the next day.
