The treatment itself is completely painless. Root canal treatment is often misunderstood to be painful because people associate it with the infection that brought them in — and that infection is exactly what the procedure removes.

Where the reputation comes from

Almost nobody arrives for a root canal feeling fine. They arrive after days of throbbing, a disturbed night, a face that aches when they lean forward. Then the treatment happens and the pain stops.

The memory afterwards merges the two — the agony of the week and the appointment that ended it — and the appointment inherits the reputation. That is the whole origin of the myth.

What the treatment does

A tooth is not solid. Inside it is a canal containing nerve and blood supply. When decay reaches that canal, the tissue inside becomes infected, and infection in a sealed space under pressure is one of the more memorable pains a person can experience.

Root canal treatment cleans the infection out of that canal and fills it with an inert material, so nothing can re-colonise the space. That is the entire principle: remove what is infected, seal what remains.

Why saving the tooth matters

Root canal treatment exists to save a natural tooth that would otherwise have to be removed, and there is no alternative treatment that achieves that. Every other route ends with the tooth gone.

An extraction is quicker and cheaper on the day. What follows is not: a gap changes how you chew, the neighbouring teeth drift, the bone beneath begins to thin, and replacing the tooth properly means an implant or a bridge. Keeping your own tooth is almost always the cheaper decision measured across a decade.

    The canal inside a tooth is narrow, curved, and not the same shape in any two people. Cleaning it thoroughly is the part that decides whether the treatment lasts. Rotary endodontics uses powered, flexible instruments to shape and clean the canal far more precisely than hand instruments allow — which means a more predictable result and less time in the chair.

    Lasers can also be used during root canal treatment, one of several procedures where they reduce discomfort.

    What to do if a tooth is hurting now

    Pain that wakes you at night, or that lingers after something hot or cold, is not the kind that resolves on its own. Painkillers manage the symptom while the cause continues. If a tooth has been sore for more than a day or two, have it looked at — the difference between a root canal and an extraction is often just how long it was left.
