A knocked out tooth

This is the one where minutes genuinely matter, and most people do the wrong thing.

  • Pick the tooth up by the crown, the part you chew with. Never touch the root.
  • If it is dirty, rinse it briefly in milk or saline. Do not scrub it and do not use soap.
  • If you can, push it gently back into the socket and bite on a clean cloth to hold it.
  • If you cannot, keep it in milk, or in the patient’s own saliva inside the cheek. Not in water, and not dry in a tissue.
  • Get to a dentist within the hour. A tooth replanted quickly often survives.

Baby teeth are the exception. A knocked out milk tooth should not be pushed back in, because it can damage the adult tooth forming above it.

A broken or chipped tooth

Rinse with warm water. Use a cold compress on the outside of the cheek to limit swelling. Keep any fragment, because a large piece can sometimes be bonded back. If the break is bleeding from the middle of the tooth, the nerve is exposed and it needs same day attention.

Swelling in the face or gum

Take this seriously. Facial swelling usually means an abscess, and infection in the lower jaw can spread to the floor of the mouth and the airway. If the swelling is closing your eye, crossing the midline of your face, or you are struggling to swallow or breathe, go to a hospital emergency department, not a dental clinic. Anything short of that, come in the same day.

Bleeding that will not stop after an extraction

Roll a clean gauze or cotton pad, place it directly over the socket, and bite firmly for twenty minutes without checking it every two minutes. Checking is what keeps it bleeding. Stay upright, avoid hot drinks, do not rinse, and do not smoke. If it is still bleeding heavily after that, call.

Lost filling or crown

Rarely an emergency, usually just uncomfortable. Keep the crown, do not glue it back with household adhesive, and avoid chewing on that side. Come in within a few days before the tooth shifts and the crown stops fitting.

Braces wire digging in

Cover the end with orthodontic wax, or a small ball of sugar free chewing gum if you have no wax. Do not cut the wire yourself. Come in and it takes a few minutes to fix.

Toothache that is keeping you awake

Pain that wakes you at night, or that lingers for more than a few seconds after something hot or cold, usually means the nerve is involved. That is a root canal conversation rather than a filling. Painkillers will get you through the night, but the pain returning is not a sign it has healed.

When to go to a hospital instead

Go to a hospital emergency department, not a dental clinic, if there is difficulty breathing or swallowing, swelling spreading down the neck or closing an eye, a suspected broken jaw, or heavy facial trauma. A dental chair is the wrong place for any of those.
